Due Date: The Health Professions Advisory Committee on-line application is due by NOON on the FIRST MONDAY of MARCH.
The Health Professions Advisory Committee application consists of the following sections:
- Personal Information
- Standardized Tests
- GPA
- Work/Activities
- Essay
- Acknowledgements
In case anything should happen while you are filling out this online form, please save the information in a separate document (especially your “Essay”) before proceeding. It is recommended that you complete this online form by copying and pasting information from a separate text only file like Notepad, TextEdit or Wordpad. Do not copy and paste directly from Microsoft Word.
You will have the opportunity to save the final version of your application as a PDF for your own records.
Task 1
You must complete the “First Name” and “Last Name” fields in the “Personal Information” section and select the "Save" button in the upper right corner of the page in order to save a draft application that you may then complete in multiple working sessions.
Task 2
General Instructions for Completing the Application
Whenever you enter or edit information, you must select the ”Save” button to save your work.
The blue arrows indicate required fields that must be completed in order to submit your application to the committee. If you missed any required fields, the form will display a notice message listing the missing required fields when you save your work and an error when you attempt to submit your application.
Please note that some of the required fields use a specific format for the information - phone numbers and dates for example. Instructions for all special fields are included with a (?) or below each field.
Clicking on the question mark (?) above some fields will provide instructions specific to that field.
When you are ready to submit your application, click on the "Save" button, click on the "Edit" tab, change the status from “Draft” to “Submitted” and click on the ”Save” button. Do not submit the application until you have completed and carefully reviewed all content.

No changes may be made after the application is officially submitted. You will receive a confirmation message via email that the application has been successfully submitted.
